New Repeat Prescription Line

From Monday 22nd October, repeat prescriptions can be ordered via our new Repeat Prescription Request Line on 01563 599300.

We will no longer be taking repeat prescription requests on our main line.

The Repeat Prescription Request Line allows you to leave a message day 24 hours a day.

Please ensure you leave the following information on our answering machine:

Your Name, Address, Date of Birth, Contact Telephone Number, Full description of the items you require.

If you telephone our main line, you will be asked to redial the above number.

This allows the practice to free up the lines for those requiring more urgent assistance.

Registrations  


Any patient wishing to register with the practice has the right to do so if they fall within our catchment area (please click below on 'Practice Boundary' to view our catchment area.

The following procedure applies:

Patients should provide their medical card and complete a 'New Patient Information Sheet'. If a medical card is not available a registration form will have to be completed.

Practice Boundary

Separate registration forms are needed for each member of a family .

Patients must produce two forms of identification. One should be photographic (passport or driving license) and the other should be a recent utility bill as proof of address.

If a Patient has a problem with identification then the matter will be passed to the Practice Manager for approval.

Once the registration form has been completed and signed and the proof of identity has been confirmed the Patient is registered with the practice and can make a normal appointment with a clinician (Once their details have been updated on the computer system - this can take up to 5 working days).   Please ensure sufficient prescription medication has been obtained from your previous practice before registering with us.

If the Patient cannot produce proof of identity at the time of completion of the forms they will be given back the registration form and asked to return with the proper documents at a later date. The Patient can leave the surgery with the registration forms.
