Any patient wishing to register with the practice has the right to do so if they fall within our catchment area (please click below on 'Practice Boundary' to view our catchment area).
The following procedure applies:
Patients should complete our patient registration papers and bring these into the practice.
YOU CAN DOWNLOAD THE REGISTRATION FORMS BELOW - PLEASE NOTE BOTH PART 1 & PART 2 ARE REQUIRED SO PLEASE DOWNLOAD BOTH AND COMPLETE.
Separate registration forms are needed for each member of a family.

When attending at the practice to hand in the registration forms, where possible, patients should produce two forms of identification. One should be photographic (passport or driving licence) and the other should be a recent utility bill (dated within last month) as proof of address.
If a Patient has a problem with providing identification then assistance will be provided.
Once the registration form has been completed and signed and the proof of identity has been confirmed the Patient is registered with the practice and can make a normal appointment with a clinician (Once their details have been updated on the computer system - this can take up to 5 working days). Please ensure sufficient prescription medication has been obtained from your previous practice before registering with us.
